Graduation Packet Paperwork

 

(The following forms are all part of the graduation packet. See “graduation procedure” for a step by step description of the graduation process that must be completed for each graduating student at the beginning of their last semester.)

 

According to the date assigned in the Paperwork Timetable, the following items should be submitted to the ES Advisor for each graduating student:

 

Graduation Packet:

-Graduation/Completion Packet Checklist

-Student Graduation Approval Request

-Graduation Requirements Worksheet

-Complete Transcript with credit summary at bottom (must include all semesters, including last, on a LVCA school transcript)

-Drop Form

-Demonstrations (if applicable)

-Work Samples (for December Grads)

 

The below items may be sent after the grad packet has been mailed, but will be considered a necessary part of a complete grad packet:
E-mail that your student has completed the final courses listed on the transcript, (include any changes that need to be made to the transcript at this time).
(Incomplete grad packets, or grad packets of students that have decided to remain in school for the next semester, will be returned to the ES).

 

Student Graduation Approval Request:

As part of the graduation process, a student/parent chooses one of the following endorsements:

 

*General Studies Endorsement- This endorsement is for students who earn Carnegie units, but did not complete the additional requirements needed to attend a four year university of their choice after graduation. If a student/parent chooses this endorsement, the box is marked and no other additional endorsement paperwork is required.

*College/University Prep Endorsement- If the parent/student chooses this endorsement, there is an additional page that must be added to the grad packet and should be filled out based on the requirements of the four year college that the student is intending to enroll in. Any courses that the college/university of their choice requires, should be listed in general on this form, with the specific course titles listed at the bottom.

*Vocational Endorsement- Students in vocational studies complete the Vocational Endorsement Packet to meet the requirements of this endorsement. It is to be included with the graduation packet. It is lengthy and will take a lot of energy, work, and commitment from the student, and should be started at least 2 years prior to the anticipated date of graduation. This endorsement takes the place of the 60 required elective credits in the special interest requirement. Schools that require Carnegie Units will not honor these credits as transfer classes nor will they count as college entrance requirements. They will only count towards our high school graduation requirements.

 

If Graduation Requirements are met, but the student does not pass both sections of the CAHSEE, they will receive a Certificate rather than a diploma beginning 2006 school year.

(Be sure that the student knows that the way their name is printed on the Student Graduation Approval Request form is the way that it will appear on their diploma or certificate of completion

The address on this form should be updated if needed before turning it in, as this is the address that the diploma or certificate of completion will be mailed to.)

 

Graduation Requirements Worksheet

This form is part of the Graduation Packet, and is used for tracking the student’s completion of the requirements for each growth area. The ES begins this form when a high schooler is first assigned to them and keeps this form updated and in the student's folder as the student completes their courses each semester.

It is helpful for the ES to complete this form on a regular basis (each semester) to make sure that the student is "on track" and will be able graduate within the agreed upon time frame. It can be quickly compared to the transcript credit summary to be sure all courses taken are on the transcript.

 

Transcript information from transferring high school students is added onto the Graduation Requirements worksheet by the ES. The information on the transcript from the prior school is important in planning classes if the student is interested in earning a high school diploma with the Charter School. As soon as the official transcript for this student is in the database, compare the info you were given by the student/parent to those courses on the transcript in the database.

 

Keep an updated copy of the Graduation Requirements Worksheet in the file you keep on each potentially graduating high school student. This student file could also include other pertinent graduation documentation you have acquired while serving the student: current transcripts, graduation requirements agreements, copies of any graduation paperwork submitted to the office, etc. Keeping your information current on a student will make any transfer to another ES a smooth process.

 

Complete/Final Transcript:

According to the date in the paperwork timetable, you will be submitting “estimated” grades for your graduates early in the semester of their graduation. These are submitted electronically and will become a part of the transcript. This will enable you to print the complete transcript for the grad packet. (If at the end of the semester the student did not meet your expectations, the grades on the transcript can be changed with a transcript form).

The complete/final transcript must have any previous school’s transcripts entered by LVCA, and must include all coursework entered on the Graduation Requirements Worksheet. The last semester’s work MUST be on this transcript.

 

Drop Form:

This drop form will be mailed to student records by the LVCA office once the ES advisor approves and turns in the completed Graduation Packet. If the Graduation Packet is incomplete, or if the student has decided to return to school the next semester, the ES advisor will return the whole packet to the ES, and the student will not be dropped until the packet is approved.
